Northwell Health Opportunity: Apply for the Northwell Advancing Age-Friendly Care Collaborative
Northwell Health is inviting applications from U.S. hospitals to participate in it's Advancing Age-Friendly Care Collaborative, a year-long experiential consulting and training program.
Funded by The John A. Hartford Foundation (JAHF), this national collaborative will guide selected hospitals in advancing the Age-Friendly Health Systems (AFHS) 4Ms Framework, with a special focus on patient experience outcomes in partnership with Press Ganey.
This is a unique opportunity for five hospitals outside of Northwell’s 28-hospital network to:
- Learn directly from Northwell Glen Cove Hospital, a nationally recognized Age-Friendly leader
- Receive expert coaching and mentorship from Northwell faculty
- Access proven, evidence-based strategies and interventions grounded in the 4Ms Framework
- Join a cohort of forward-thinking organizations committed to transforming care for older adults
