AARP/JAHF Webinar Recording: Why Family Caregivers Need Support in Care Transitions

AARP webinar5

AARP and JAHF hosted a webinar discussion with healthcare leaders innovating on hospital front lines in support of family caregivers, March 24, titled "Why Family Caregivers Need Support in Care Transitions."

Family caregivers, patients, and providers all play a role in maintaining a patient’s health after hospital discharge. Yet, the way post-hospital transitions are handled can create difficulties and put a significant responsibility on the family caregiver - whether the discharge is to home, a rehab facility, assisted living, or a nursing home.

The webinar was for healthcare leaders, hospital and system executives, and those with influence over the patient experience. The event's aims were:

  • Better understand what family caregivers need related to discharge and the eventual transition from the hospital
  • Consider how age-friendly policies and practices can impact successful discharge and follow-up care
  • Drive proactive connections to community resources that build equity in the services and supports family caregivers need most

The webinar was hosted by:

  • Susan C. Reinhard, PhD, RN, FAAN, Senior Vice President and Director, AARP Public Policy Institute & Chief Strategist, Center to Champion Nursing in America
  • Rani Snyder, MPA, Vice President, Program, The John A. Hartford Foundation

Panelists included:

  • Karen Ortiz, Outreach & Family Caregiver Support Network Coordinator, Lompoc Healthcare District, Lompoc Valley Medical Center
  • Anita Girard, DNP, RN, CPHQ, NEA-BC, Vice President, Nursing, Cedars-Sinai

There was dedicated time for live Q&A.
