AARP has published videos from health professionals who have implemented the Age-Friendly Health Systems movement's My Health Checklist. The videos are part of AARP's initiative, "What Matters to Older Adults."

My Health Checklist is a guide that uses the Age-Friendly Health Systems 4Ms Framework to help older adults identify concerns that are most important to them and prepare to share those concerns during medical appointments.

Recently, AARP and the Institute for Healthcare Improvement (IHI) led a community of practice composed of a dozen groups working to use My Health Checklist. Participants workshopped ways to introduce the resource to team members, as well as to patients and their caregivers.

As a leader at one participating organization noted, “My Health Checklist, for me, is just a wonderful tool to help individuals prepare for shared decision-making.” Another spoke about the importance of age-friendly health care, saying, “It’s addressing the ageism, the implicit bias that may get in the way of us hearing older adults when they are trying to express their needs.”

The videos offer additional perspectives from health professionals who have implemented the My Health Checklist:
