Age-Friendly Health Systems Case Studies: Spreading 4Ms Care Across the Health System
The Institute for Healthcare Improvement (IHI) has released new resources from the Age-Friendly Health Systems (AFHS) System-Wide Spread Collaborative, including two case studies from Collaborative members.
The latest case study, "System-Wide Spread of the 4Ms at Bassett Healthcare Network," highlights lessons from spreading 4Ms care across a smaller, largely rural health system, including hospitals, outpatient practices and long-term care settings.
The previous case study, "System-Wide Spread of the 4Ms at MediSys Health Network," shows how MediSys successfully scaled the 4Ms framework across its entire system, improving consistency and outcomes in care for older adults.
IHI also released a video, "Spreading 4Ms Care Across the Health System," explaining how health systems across the U.S. are adopting the AFHS initiative to improve care for older adults.
