Age-Friendly Public Health Systems Resource: 6Cs Training and Implementation Guide 2025

Trust for America's Health (TFAH) has published a resource, Age-Friendly Public Health Systems 6Cs Training and Implementation Guide. This framework equips public health systems to better support the growing older adult population through coordinated, equitable and age-friendly approaches.

The guide introduces the 6Cs Framework, a set of six core public health roles that support healthy aging across communities, which include:

  1. Creating and leading policy, systems, and environmental changes
  2. Connecting and convening multi-sector partners
  3. Coordinating existing supports and services
  4. Collecting, analyzing, and translating relevant data
  5. Communicating important public health information
  6. Complementing existing health promoting program

Developed by TFAH in partnership with the Association for State and Territorial Health Officials (ASTHO), the National Association of County and City Health Officials (NACCHO), and the National Network of Public Health Institutes (NNPHI), the 6Cs Framework serves as a practical roadmap for integrating age-friendly principles into public health infrastructure.

In a recent interview, TFAH's Megan Wolfe shares insights on how the guide was developed and how public health departments can apply it to strengthen healthy aging strategies. She highlights features such as module-specific resource lists and reflection tools designed to support real-world application and continuous learning.
