Aging and Disability Business Institute Releases Two New Infographics
The Aging and Disability Institute released two new infographics that explore how Area Agencies on Aging (AAAs) and other community-based organizations (CBOs) address the social determinants of health of older adults and people with disabilities in their communities through contracts with health care entities.
The infographics identify key partners and services in these contracts, as well as where this contracting activity is occurring around the country—and how this work has evolved over time.
AAAs Address Social Needs Through Contracts with Health Care
Established as part of the Older Americans Act (OAA) in 1973, Area Agencies on Aging (AAAs) are charged with meeting the needs of Americans 60 and older in local communities by developing, coordinating and delivering a wide range of home and community-based services. Health care payers and providers are increasingly contracting with AAAs to address the social determinants of health among their clients. Given their nearly 50-year history as the local leaders on aging well at home and in the community, AAAs have long addressed the social determinants of health by providing home and community-based services.
CBOs Address Social Needs Through Contracts with Health Care
Aging and disability community-based organizations (CBOs), such as Area Agencies on Aging, Centers for Independent Living and other service providers, provide a wide range of home and community based services to help older adults and people with disabilities live with dignity and independence in their homes and communities for as long as possible. Health care payers and providers are increasingly contracting with CBOs to address the social determinants of health among their clients by providing home and community-based services.
