Aging and Disability Business Institute Success Stories: Case Study Series
The Aging and Disability Business Institute (Business Institute) at USAging has released a case study series on building collaborative contracts with health care, highlighting the work of The John A. Hartford Foundation 2023 Business Innovation Award winners. Two new case studies were recently added featuring the award runners-up.
The first case study is from Western New York Integrated Care Collaborative (WNYICC) - the 2023 Award winner - for its work as a Community Care Hub to address the social determinants of health. Through WNYICC's 25 contracts with health plans, it's more than 50 members help reduce barriers older adults and people with disabilities may face in accessing medical care by addressing the health-related social needs of individuals living in western New York through a variety of programs, making a profound difference in the lives of over 1,500 individuals in this region.
The second case study is from 2023 JAHF Business Innovation Award runner-up, Region IV Area Agency on Aging (Region IV AAA) in St. Joseph, MI, on how it has built a collaborative contract with Corewell Health to provide services to older adults with complex needs. This has resulted in reductions in unplanned hospitalizations, length of hospital stays, and emergency department transitions to nursing homes with $1.7 million in cost savings after 6 months.
The third case study is from 2023 JAHF Business Innovation Award runner-up, Central Ohio Area Agency on Aging (COAAA) in Columbus, OH, on its contracts with Molina Healthcare of Ohio and Aetna Better Health. The COAAA was recognized for creating a simplified tool that allows community health workers, social workers and registered nurses to screen patients for health-related social needs.
