Business Institute Success Story Case Study: Building Collaborative Contracts with Health Care with Direction Home Akron Canton and Medical Mutual of Ohio
The Aging and Disability Business Institute (Business Institute) has released a case study on "Building Collaborative Contracts with Health Care: Direction Home Akron Canton and Medical Mutual of Ohio."
This latest publication in their Success Story series reports on the success of the Direction Home Akron Canton Area Agency on Aging & Disabilities (DHAD) Transitional Care Program. DHAD, along with Medical Mutual of Ohio, partnered with Ohio’s Area Agencies on Aging (AAAs) to support older adults and people with disabilities as they transitioned home after hospital stays and were able to serve over 3,100 Medical Mutual of Ohio members annually.
The case study describes the program and its impact, gives insight into how the contract between DHAD and Medical Mutual of Ohio was developed and sustained, and details the successes and challenges of health care contracting.
DHAD received The John A. Hartford Foundation Business Innovation Award in 2022 for their outstanding model of care.
