Aging and Disability Business Institute Toolkit: The Health Care Guide to Contracting with CBOs
The USAging’s Aging and Disability Business Institute (Business Institute) has partnered with the Partnership to Align Social Care and the Camden Coalition on a new resource toolkit, "The Health Care Guide to Contracting with Community-Based Organizations (CBOs)."
The four-part toolkit contains extensive input from CBOs, health plans, and health systems, and focuses on the essential elements of contracting between health plans and community partners and features leading practices from the field.
Each part of the toolkit addresses a critical component of the contracting process:
- Part One: Addressing the Complex Needs of Older Adults through Strong Community-Based Organization and Health Plan Partnerships
- Part Two: Designing the Payment Structure: A Health Plan’s Guide to Paying CBOs & CCHs for Social Care (October 2023)
- Part Three: Defining Core Activities: Developing an Effective Scope of Work For Contracted Partners
- Part Four: Operationalizing Contracts: Improving Contracting Implementation and Collaboration
- Resource Guide
- Joint Operating Committee Template (developed by the Camden Coalition)

While primarily designed for a health care audience, community-based organizations, community care hubs, and other stakeholders in the health and social care ecosystem can benefit from understanding how health care organizations approach the contracting process.
