AHA Age-Friendly Healthcare Impact Award: Applications Open
The new AHA Age-Friendly Healthcare Impact Award, presented by the American Hospital Association and sponsored by The John A. Hartford Foundation, recognizes hospitals and health systems that are transforming care for older adults.
Eligibility is open to U.S. hospitals and health systems that have at least two settings of care where the 4Ms (What Matters, Medication, Mentation and Mobility) are being delivered and have earned level 2 recognition as an “Age-Friendly Health System — Committed to Care Excellence.”
Applications for the inaugural award are open now and are due September 25.
