A Clinical Pathway to Well Being Putting Patient Priorities at the Center of Care

The American Journal of Accountable Care has published an article, "A Clinical Pathway to Well-Being: Putting Patient Priorities at the Center of Care."

The article focuses on Devoted Medical's Patient Priorities Care model, a clinical framework that delivers personalized care by prioritizing what matters most to the patient, their needs and goals. Devoted Medical used this framework with 124,000 Medicare Advantage patients and found it a powerful motivator for improving health outcomes.

By prioritizing patient-defined goals and implementing the Patient Priorities Care framework, Devoted Medical saw substantial improvements in clinical outcomes for patients with type 2 diabetes, congestive heart failure and hypertension.

This initial evidence suggests that implementing the Patient Priorities Care clinical framework in a commercial setting can realign care systems around the priorities of older adults, resulting in better health outcomes.
