American Society on Aging Webinar: My Health Priorities - Provider Training
The American Society on Aging (ASA) is holding a live virtual provider training on October 16 on Patient Priorities Care's resource, My Health Priorities.
The course will introduce the My Health Priorities (MyHP) resource which helps older adults identify what matters most to them regarding health, life goals and care preferences. MyHP supports the nationwide effort of the Age-Friendly Health Systems 4Ms Framework (What Matters, Medication, Mentation, and Mobility). Case studies will be used to highlight how MyHP can improve healthcare and social services delivery, as well as older adult physical and mental health and quality of life.
The training will be led by Claire Davenport, MD, a geriatrician and internist with national leadership experience in Patient Priorities Care implementation.
The course is for older adults and care partners, geriatric care managers, social workers, healthcare advocates, healthcare providers, case managers, senior living staff, homecare organizations, aging services professionals, and patient navigators. The virtual training will cost $35 for ASA members and $45 for non-members.
