Amy Berman to Keynote Greater Cincinnati Health Council Advance Care Planning Coalition Meeting
Amy Berman, Hartford Foundation Senior Program Officer, will be keynoting the Greater Cincinnati Health Council’s Advance Care Planning Coalition kick off meeting on August 26-27, 2014. Approximately 9-10 hospitals are anticipated to participate in the 9-month Advance Care Planning Coalition effort. Each hospital will choose at least one care continuum partner—from long term care, home health, physician practices, or community-based services. The goal of the initiative is to recognize, honor and support the needs, wishes and goals of frail elders with advanced illness in the community. The effort is being organized by Margie Namie, past participant in the Hartford Foundation’s Practice Change Leaders Program and a former Innovation Advisor to the Centers for Medicare and Medicaid Innovation.
