The Centers for Medicare and Medicaid Services (CMS) Medicare-Medicaid Coordination Office, in collaboration with Community Catalyst and the American Geriatrics Society - both JAHF grantees - and The Lewin Group, are hosting a Geriatric Competent Care Webinar.
Discussion will include the business case for a dementia-capable delivery model of care, key components of an effective care model, and interventions implemented within two Medicare-Medicaid financial alignment model demonstrations.
