The article covers the announcement by The John A. Hartford Foundation of funding approved at the September Board of Trustees meeting in support of three initiatives dedicated to improving the care of older adults.
"The awards include a grant of more than $2.6 million over three years to the American Geriatrics Society, which will use the funds to improve health outcomes for older adults by integrating Age-Friendly Health Systems principles into its Geriatrics Workforce Enhancement Program, and a grant of more than $2.5 million over three years to the Henry J. Kaiser Family Foundation's Kaiser Health News service in support of its efforts to boost knowledge of older adults' health care issues among policy makers, health systems, and the public...In addition, the foundation awarded more than $1.5 million over three years to Massachusetts General Hospital, the Home Centered Care Institute, and the American Academy of Home Care Medicine for phase two of an initiative to improve the health of frail and vulnerable older adults by increasing access to high-quality, home-based primary care."
