Medical Care has published a commentary, "Should We Keep Spackling the Cracks Across the Health Care Continuum or Build a New Path Forward With Safer Transitions of Care?" by JAHF Senior Program Officer Amy Berman, RN, LHD (hon), FAAN.

"Transitions in care are rife with hazard, the dangerous cracks in health care delivery," says Dr. Berman, and speaks on the need to address the medical systems' shortcomings in transition of care, discussing initiatives that are working to address these cracks. She talks about the Patient-Centered Outcomes Research Institute (PCORI) Transitional Care initiative along with other transitions of care projects and models supported by JAHF.

"As we all address the many problems that cause preventable harms during transitions of care," Dr. Berman notes, "we also need to rid health care of the underlying and pervasive cracks along the path," and cites the Age-Friendly Health Systems initiative. "What makes Age-Friendly Health Systems of note is that it transforms the delivery of care reliably and consistently across settings and providers of care."
