ASA Articles on Age-Friendly Health Systems
The American Society on Aging (ASA) Aging Today has published two articles in it's March/April issue about Age-Friendly Health Systems.
These are part of a series of Aging Today articles and case studies by ASA and the National Association of Area Agencies on Aging's (n4a) Aging and Disability Business Institute (ADBI) that highlight the Age-Friendly Health Systems initiative.
The articles are:
- My Story: A Simple But Powerful Tool to Improve Eldercare, talks about a collaboration between the Anne Arundel County Department of Aging and Disabilities and the Anne Arundel Medical Center (AAMC), both in Annapolis, MD.
- The Community Care Settings Pilot: Ensuring A Smooth Transition Back Into Community, talks with San Francisco’s Institute on Aging (IOA), winner of The John A. Hartford Foundation 2019 Business Innovation Award. The IOA received this award for its pioneering work in helping older adults and individuals with disabilities to transition from institutional settings into living independently in the community.
