The Better Care Playbook is holding a webinar June 13 on "Addressing the Complex Needs of Older Adults through Strong Community-Based Organization (CBO) and Health Plan Partnerships."
Health care entities, CBOs and community care hubs are increasingly partnering to deliver home- and community-based services and other evidence-based services for older adults, many of whom have chronic health conditions and disabilities. The webinar will feature perspectives from program leaders and researchers on the value case for partnership, describe different partnership models, and highlight new resources to facilitate effective contracting to support adults of any age. It will explore how one successful model entered into a partnership, how they put their contract into practice, and outcomes of the partnership to date.
