The Blueprint for Complex Care Stakeholder Survey
The Blueprint for Complex Care stakeholder survey is a joint project of the National Center for Complex Health and Social Needs, the Institute for Healthcare Improvement (IHI), and the Center for Health Care Strategies (CHCS).
The National Center, an initiative of the Camden Coalition of Healthcare Providers, was launched in 2016 with the aim of improving outcomes for patients with complex medical, psychological, and social needs.
The Blueprint will serve as a strategic plan for the emerging field of complex care, to be shared in the fall of 2018. The 12-question survey seeks input from stakeholders on their ideas about complex care.
