The BOLD Public Health Center of Excellence on Dementia Caregiving (PHCOE-DC) is holding a webinar on December 9, "Building Systems for Comprehensive Dementia Care: Opportunities for Public Health to Support the Implementation of the CMS GUIDE Model."
Webinar speakers from Tennessee and Rhode Island will discuss the CMS GUIDE Model and how public health agencies can support its local implementation, with a focus on family caregiver support and education. Participants will discover ways to connect with health systems in the GUIDE pilot, build community-clinical partnerships, and enhance dementia care for underserved communities while gaining insights and resources to help develop a supportive infrastructure for comprehensive dementia care.
