The Coalition to Transform Advanced Care (C-TAC) held a webinar on March 9, "Understanding and Respecting What Matters Most to Patients and Family Caregivers."

In October 2019, WebMD and The John A. Hartford Foundation published findings from an online survey of older adults and caregivers on their attitudes and experiences about the "4Ms" of age-friendly health systems.

68% of patients said they initiated conversations with their doctor about what matters to them. During the webinar, experts in advance care planning and serious illness communication discussed the findings and talked about strategies for leading effective conversations with patients and caregivers. Speakers included:

  • Marian Grant, DNP, ACNP-BC, ACHPN, FPCN, RN, Senior Regulatory Advisor, C-TAC (Moderator)
  • Amy Berman, RN, LHD, FAAN, Senior Program Officer, The John A. Hartford Foundation
  • Carole Montgomery, MD, Executive Medical Director, Respecting Choices®
  • Anna Gosline, Executive Director, Massachusetts Coalition for Serious Illness Care
