CAPC Publication: Palliative Care Is Essential for Age-Friendly Care for Older Adults with Serious Illness

CAPC Position Statement

The Center to Advance Palliative Care (CAPC) has released a new publication, "Palliative Care Is Essential for Age-Friendly Care for Older Adults with Serious Illness.”

The paper outlines why palliative care is essential to the goals and measures of Age-Friendly Health Systems, and explores how health care organizations can integrate palliative care teams and training initiatives to optimize age-friendly care for older adults with serious illness.

It also explains how palliative care aligns with the 4Ms Framework of Age-Friendly Health Systems (What Matters, Medication, Mentation and Mobility) and describes how palliative care teams and workflows can support Age-Friendly Health Systems recognition and performance on the Centers for Medicare & Medicaid Services Age Friendly Hospital Measure.

CAPC also offers recommended actions for hospital leaders, policymakers and age-friendly care teams to better integrate palliative care into quality improvement efforts.
