The article highlights how SSM Health first piloted the Age-Friendly Health Systems 4Ms Framework (What Matters, Medication, Mentation and Mobility) at St. Mary’s Hospital – Madison and then expanded it across all seven hospitals in Wisconsin, with plans to spread further in Missouri, Illinois and Oklahoma. Leaders describe how the model breaks down silos across care settings and has become a “playbook” for delivering patient-centered care.
The article details how SSM Health integrated the 4Ms into daily practice through tip sheets, daily rounds and a new electronic health record dashboard. Early results show improved patient satisfaction, and leaders note the work positions the system well for the new CMS Age-Friendly Hospital Measure.
The Age-Friendly Health Systems initiative is led by the Institute for Healthcare Improvement (IHI) and JAHF, in partnership with the Catholic Health Association of the United States (CHA) and the American Hospital Association (AHA).
