Center for Health Care Strategies: Multisector Plan for Aging Toolkit

CHCS Multisector Plan for Aging Toolkit

The Center for Health Care Strategies (CHCS) has released a "Multisector Plan for Aging Toolkit."

States across the country are pursuing comprehensive, statewide multisector plans for aging (MPAs) to better coordinate policies and programs affecting older adults, people with disabilities, and their caregivers. To date, 16 states have authorized an MPA, and another 21 are actively planning or exploring one.

The toolkit - developed with support from The SCAN Foundation, West Health, the May & Stanley Smith Charitable Trust, and The Henry and Marilyn Taub Foundation - is designed to support states and their partners at every stage of the MPA process. It includes six modules — from securing leadership buy-in to sustaining progress over time — that offer practical guidance, concrete strategies and real-world examples drawn from states actively engaged in this work.
