Center to Advance Palliative Care Training: Age-Friendly Health Care Learning Pathway
The Center to Advance Palliative Care (CAPC) has released a set of training courses for an Age-Friendly Health Care Learning Pathway.
In the units, participants are introduced to concepts on how Age-Friendly Health Systems (AFHS) can deliver high-quality care and reduce harm to older adults through the evidence-based AFHS 4Ms framework (What Matters, Medication, Mentation, Mobility).
Topics include clarifying the patient's goals, managing their medications, addressing cognitive issues, and optimizing mobility by assessing and addressing their pain. This Learning Pathway provides comprehensive education on these topics, resulting in a CAPC Designation status to demonstrate age-friendly care competency.
