The Community Health Accreditation Partner (CHAP) is holding a webinar on April 26, "Age-Friendly Care at Home - Getting Certified with CHAP."
The webinar follows CHAP's announcement that home health and hospice providers now have the option to achieve a Certification for Age-Friendly Care at Home, in addition to their CHAP Accreditation. This webinar will discuss what age-friendly care is, how one can get certified during accreditation and the value this can bring to patients, providers and agencies as a whole. The webinar will cover:
- The Age-Friendly Health Systems 4Ms Framework for providing age-friendly care
- How adding this certification impacts site visits
- Review the value of having agencies be an age-friendly care provider
