Join the ONC, Hartford Foundation, Gordon & Betty Moore Foundation and Health 2.0 for a webinar on March 7th at 4 PM ET. The Office of the National Coordinator for Health Information Technology is organizing a challenge to software developers to create an easy-to-use tool that will make post-discharge follow-up appointment scheduling a more effective and shared process for providers, patients and caregivers. The challenge is part of ongoing follow-up efforts from a 2011 meeting, Putting the IT in Care TransITions, supported by the John A. Hartford Foundation, the Gordon and Betty Moore Foundation, and Kaiser Permanente in partnership with the ONC.
