The brief highlights how FAIR Health's shared decision-making tools support conversations between older adults, caregivers, and healthcare providers at the point of care. Developed as part of a national initiative, these tools combine clinical and cost information to help patients make informed decisions aligned with their values.
Insights from clinicians at four Age-Friendly Health Systems sites highlight the tools’ flexibility, value to caregivers, role in cost discussions, and support for care planning.
