GWEP-Coordinating Center: Two New Case Study Series and Age-Friendly Resources Library
The GWEP-Coordinating Center (GWEP-CC) has released two case study series, one on Sustainability and one on Age-Friendly Health Systems, and an Age-Friendly Resources Library.
These resources are free - for those who aren't AGS members, it is free to register for a GeriatricsCareOnline (GCO) account to access the case studies and resources library.
The GWEP-CC case studies series are on:
- Sustainability: This series of case studies presents business cases from the GWEPs that focus on sustainability, communication, and messaging about their GWEP grants. Case study authors are from six of the forty-eight currently funded GWEPs. The case studies provide valuable insight on sustaining a program when funding ends.
- Age-Friendly Health Systems (AFHS): fifteen new case studies have been published and added to this collection for a total of twenty-seven case studies. Case study authors participated in the 2020 and/or the 2023 GWEP-CC AFHS Action Community. IHI has recognized these practices as an AFHS Participant (Level-1) or AFHS – Committed to Care Excellence (Level-2).
The AGS Age-Friendly Resource Library, formerly known as the 4Ms Toolkit, provides resources based on the 4Ms framework of an Age-Friendly Health System from the IHI, AGS, and the AGS Health in Aging Foundation (HiAF). These resources are designed to support one’s work on the path to becoming Age-Friendly and focus on different aspects of the 4Ms. Each of the 4Ms is divided into its own section and is separated between professional and public resources. The library also includes sections on the Business Case for Becoming an Age-Friendly Health System and Cross-Cutting resources.
