The Hospital at Home (HaH) Users Group, in partnership with the American Academy of Home Care Medicine and the American Hospital Association, is holding a webinar on September 28, "By the Numbers: Financial Models, Value Propositions, and Projections for the Next Generation of HaH programs."

In this session for hospital and system leaders interested in learning about the opportunities and challenges associated with providing acute care in the home, Tom Kiesau of The Chartis Group will share perspectives on how a health system should approach developing the financial model around its hospital at home program. Moderated by Linda DeCherrie, MD, the session will:

  • Share insights on how to define explicit clinical population scoping, both at program launch, and expanding/evolving over time
  • Show how to estimate programmatic cost over a 3-5 year program planning horizon
  • Demonstrate how to quantify near and long-term economic value creation enabled by the launch of a hospital at home program
  • Share common pitfalls and mistakes health systems make when planning for hospital at home
