JAHF home-based primary care grantees have published an article, "The Power of Three Plus One: The John A. Hartford Foundation (JAHF) Expands Collaborative Grant Effort to Advance Home-Based Primary Care."

Moving and Scaling Home-Based Primary Care Phase III: Quality, Training, Advocacy, and Policy is the third phase of the collaborative initiative and involves four organizations:

  1. The National Home-Based Primary Care Learning Network
  2. The Home Centered Care Institute (HCCI)
  3. The American Academy of Home Care Medicine (AAHCM)
  4. The Duke-Margolis Institute for Health Policy (newest addition to the initiative)

This phase builds on two previous phases of this ongoing work to advance home-based primary care in the US, with the newest addition to the group adding a focus on policy as a lever to promote home-based primary care.
