The blog highlights the growing capacity of aging and disability community-based organizations to address the social determinants of health (SDOH). Written by Assistant Secretary for Aging and ACL Administrator Lance Robertson and The SCAN Foundation President and CEO Bruce Chernof, they note that current partnerships between CBO networks and health care organizations have produced great progress in addressing the social needs of patients. The next step, the authors say, is to scale these CBO health care partnerships across the country, with shared investment from both health care and social services, and they lay out the way forward.
