Hospital at Home Users Group Guide: An Introduction to Age-Friendly Care in Hospital at Home
The Hospital at Home Users Group (HaH) has published a guide, "An Introduction to Age-Friendly Care in Hospital at Home."
This guide provides an overview of age-friendly care concepts, including the 4Ms, and how to integrate age-friendly care in the HaH setting. It also provides information for how HaH care sites can be recognized as age-friendly. This resource was created through contributions from the Cincinnati VA, Saint Luke’s Hospital of Kansas City, Kent Hospital in Rhode Island, the Complex Care Hub at the University of Calgary, Canada, and additional members of the HaH Users Group in collaboration with JAHF.
HaH has also published a resource on "Accommodating Patients with Hearing, Vision, Speech or Language Disabilities." Developed with the Disability Equity Collaborative, this outlines key considerations, best practices, and ideas for providing accommodations for patients living with certain types of disabilities.
