Hospital at Home Users Group Webinar Recording: Finding Patients Faster - Early Transfer Recruitment Strategies for HaH
The Hospital at Home (HaH) Users Group, in partnership with the American Academy of Home Care Medicine (AAHCM), held a webinar January 27 on, "Finding Patients Faster: Early Transfer Recruitment Strategies for HaH."
This webinar was designed for hospital and system leaders interested in learning about the opportunities and challenges associated with providing acute care in the home.
Increasing early transfer admissions from EDs is an essential mechanism for growing HaH programs to scale. These admissions maximize HaH’s benefits for patients and health systems and result in a better care experience, reduced strain on ED capacity, and better financial returns for HaH care.
Webinar speakers explored common challenges to early transfer recruitment and discussed the strategies they have employed to improve recruitment in their programs. Speakers included:
- Kamia Thakur, MD, medical director, HaH, Penn State Health
- Rajan Gurunathan, MD, FACP, vice president chief medical officer, HaH, Hackensack Meridian Health
- Leah Webster, PharmD, BCPS, clinical pharmacy coordinator, Advanced Care at Home, Mayo Clinic
- Moderator: Jay Mathur, MD, regional medical director, Contessa Health
