IHI Age-Friendly System-Wide Spread Collaborative Resources: 4Ms System-Wide Spread Guide and Tools
The Institute for Healthcare Improvement (IHI) has released new Age-Friendly System-Wide Spread Collaborative resources including a 4Ms System-Wide Spread Guide and other tools.
The collaborative brings together health systems to accelerate and spread adoption of 4Ms care for older adults across all of their sites and care settings.
Newly released resources for health systems to support their 4Ms System-Wide Spread include an accessible guide to what drives system-wide spread, a planning tool, a readiness assessment and a video case study:
- Spreading Age-Friendly Care: Drivers of Successful Health System-wide Spread of the 4Ms: Guide describing the drivers of spread of the 4Ms through all sites and settings of care in a health system, including examples of key changes systems can test and implement.
- Age-Friendly Care System-wide Spread Plan: Tool to help teams articulate a vision for the spread of the 4Ms across the health system and document those actions.
- Age-Friendly Care System-wide Spread Self-Assessment: Tool designed to help systems identify strengths and opportunities as they relate to the key drivers of spread.
- Video Case Study from Grady Health: The video explores the system’s approach to spreading age-friendly care.
