IHI and TFAH Webinar Recording: Three Keys to Cross-Sector Age-Friendly Care

The Institute for Healthcare Improvement (IHI) and Trust for America’s Health (TFAH) held a webinar December 5 on, "Three Keys to Cross-Sector Age-Friendly Care."

The Three Keys resource is designed to foster collaboration across the older adult care continuum, promoting system changes that will lead to an optimal care journey for older adults and their caregivers. The resource includes concrete guidance for health systems and public health agencies, as well as community organizations to identify and prioritize activities in healthy aging.

The webinar covered the development of related resources and the value of implementing the change ideas designed specifically for each sector. It also provided an opportunity to share feedback on the Three Keys resource.

This effort was the result of a collaboration between IHI, TFAH and the Michigan Health and Hospital Association, with funding from the Michigan Health Endowment Fund.
