IHI Informational Call Recording: Age-Friendly Health Systems Action Community
The Institute for Healthcare Improvement (IHI) held an informational webinar on February 10 that gave a high-level overview of the upcoming March 2023 IHI Action Community.
Designed to help patient care teams adopt age-friendly care in hospitals, intensive care units, ambulatory settings, nursing homes and convenient care clinics, the IHI Action Community is a virtual learning opportunity to orient health systems and clinical practices towards an age-friendly framework. Over the course of seven months, participating teams will engage in peer-to-peer and coaching webinars to understand and implement 4Ms (What Matters, Medications, Mentation, and Mobility) in respective care settings.
The Action Community is guided by expert faculty and an “all teach all learn model,” providing opportunities for teams to share challenges and successes of implementing age-friendly care with each other.
