IHI New Resources for Providers My Health Checklist

The Institute for Healthcare Improvement (IHI) has released new resources for providers on using My Health Checklist with older adults.

My Health Checklist is a workbook that helps people age 65 and older prepare for health care appointments by identifying the concerns that matter most to them in the 4Ms Framework of age-friendly care (What Matters, Medication, Mentation and Mobility).

The provider resources include guidance on introducing the workbook to older adults, caregivers and staff, along with tips and stories from the field and a sample presentation. The materials are designed to support implementation of My Health Checklist in care settings and to help providers incorporate the tool into routine practice.

These resources were supported by AARP and JAHF.
