IHI Resources to Practice Age-Friendly Care

The Institute for Healthcare Improvement (IHI) has created a resource hub to help care teams practice age-friendly care.

Age-Friendly Health Systems (AFHS) is an initiative of JAHF and IHI, in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA), designed to help care teams test and implement a specific set of evidence-based, geriatric best practices called the "4Ms": What Matters, Medication, Mentation, Mobility.
