IHI Videos: Age-Friendly Health System Video Case Study Series on Age-Friendly Care
The Institute for Healthcare Improvement (IHI), in partnership with Penn State, has released a video case study series on Age-Friendly Care.
The 3-part IHI video series was made in partnership with Penn State Geriatrics Workforce Enhancement Program (GWEP) and their Federally Qualified Health Center partner, Primary Health Network (PHN), the GWEP-CC. The series showcases how the Penn State GWEP and PHN built their Age-Friendly Health Systems team and how they tested the 4Ms Framework using small tests of change and the impact and outcomes of those tests.
The three Age-Friendly Health Systems videos in the series are:
- Getting Started with Age-Friendly Care
- Implementing the 4Ms of an Age-Friendly Health System
- Building Connections through Age-Friendly Care
This video series is based on Penn State GWEP and PHN’s case study publication that follows their Age-Friendly journey. Additionally, Dr. Laurence Solberg, a geriatrician for the Veterans Health System, talks about the impact Age-Friendly care has on his patients by sharing one veteran's story in a video interview.
