Information Session on Tool for Interoperability for Nursing Homes & Home-Health - KeyHIE Transform™

The Keystone Beacon Community, one of 17 Beacon Communities the ONC (Office of the National Coordinator for Health IT) has funded across the country to demonstrate meaningful improvements in health, care and cost, has launched a new tool (KeyHIE Transform™) for electronically sharing patient information from skilled-nursing facilities and home health agencies. The Keystone team has partnered with BridgeGate International to distribute KeyHIE Transform™ via a low-cost online service to any community across the country seeking to bring LTPAC and home health partners into community-wide care coordination efforts. The team will offer two one-hour webinars in September on KeyHIE Transform™, and invite you, your colleagues, and other stakeholders across your community to join.

The Keystone Beacon Community, one of 17 Beacon Communities the ONC (Office of the National Coordinator for Health IT) has funded across the country to demonstrate meaningful improvements in health, care and cost, has launched a new tool for electronically sharing patient information from skilled-nursing facilities and home health agencies. KeyHIE Transform™ extracts clinically relevant information from the Minimum Data Set (MDS 3.0) or the Outcome and Assessment Information Set (OASIS), which these entities are required to submit to CMS, and then converts this data into a standards-based continuity-of-care document (CCD), which can be incorporated into any health-information exchange for access by other clinicians. Keystone worked with ONC’s Standards and Interoperability Framework to develop this standard, which has been balloted with HL7 and is described in publicly available implementation guides.

  • 1:00 – 2:00 PM ET on September 5, 2013
  • 1:00 – 2:00 PM ET on September 19, 2013

During the webinar, Jim Younkin, Program Manager for the Keystone Beacon Community and Director, Information Technology, at Geisinger Health System will discuss the basics of KeyHIE Transform™, provide an update on how the tool is being used today, and share information on how interested providers can participate.

JOINING THE WEBINAR

  • To join the webinar, please log on to https://beaconcommunity.webex.com about 5 minutes before the call.
  • Under the list of events, look for “KeyHIE Transform™ Information Session”
  • Click on the “Join” link to the right of the session
  • You will then be asked to enter your email address and your name (Also enter in the Beacon Community you are from)
  • Your screen will adapt for a few minutes while the session is getting set up on your computer
  • A grey pop-up box will appear - You’ll want to select “I will call in” from the drop down box
