The Institute for Healthcare Improvement (IHI) is holding an informational call on March 1 about the next Age-Friendly Health Systems (AFHS) Action Community that starts this March 2022.
The AFHS Action Community brings teams across different health systems together to accelerate their own description and adoption of the 4Ms: What Matters, Medications, Mentation, and Mobility. Join the call to hear IHI's Age-Friendly Health Systems team and special guests talk about the next Action Community.
AFHS is an initiative of The John A. Hartford Foundation (JAHF) and IHI in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).
