Invitation to Join: 2024 Veterans Affairs Age-Friendly Health Systems Action Community

VA AFHS Action Community

The U.S. Department of Veterans Affairs (VA) is inviting VA health care teams to join its 2024 VA Age-Friendly Health Systems Action Community.

Age-Friendly Health Systems (AFHS) follows an essential set of evidenced-based practices called the 4Ms: What Matters, Medication, Mentation (Mind) and Mobility. At the core of these practices is value and respect for Veterans’ wishes and priorities with care plans based on what matters most to Veterans and their families.

In March 2020, the VA's Geriatrics and Extended Care (GEC) set the aim for the VA to become the largest integrated health care system in the U.S. to be recognized by the Institute for Healthcare Improvement (IHI) as Age-Friendly. Since then, more than 100 VA medical centers across the country have joined the movement as official Age-Friendly Health Systems Participants.

The seven-month AFHS Action Communities provide a series of monthly webinars and coaching calls to support teams on their Age-Friendly journey.

  • Kick-off Call is Wednesday, December 6 at 2pm ET
  • Monthly Webinars (second Wednesdays) at 2pm ET, January through July 2024
  • Monthly Coaching Calls (fourth Wednesdays) at 2pm ET, January through June 2024
