Invitation to Join: Age-Friendly Health Systems 2026 Action Community
The Institute for Healthcare Improvement (IHI) is inviting health professional teams to join the next Age-Friendly Health Systems Action Community, launching in spring 2026. The Action Community is for teams interested in accelerating the adoption of evidence-based age-friendly care in their setting.
Age-Friendly Health Systems (AFHS) is built on the evidence-based 4Ms Framework: What Matters, Medication, Mentation and Mobility. Over seven months, a team will test the 4Ms at their site of care (e.g., a hospital unit, primary care practice, nursing home, hospice, home health) with the goal of achieving recognition as an Age-Friendly Health System.
For hospitals, participation offers a valuable opportunity to prepare for the CMS Age-Friendly Hospital Measure.
New for 2026: Hospice organizations are now included as a participating care setting. Hospice teams will engage in tailored learning topics and peer coaching calls informed by insights and best practices from pilot testing conducted over the past year.
The Action Community includes monthly webinars, coaching calls, and access to practical tools and peer learning to help teams integrate age-friendly care into daily practice.
