Invitation to Join: HANYS 2025 New York State Age-Friendly Health Systems Action Community

The Healthcare Association of New York State (HANYS) is inviting health systems teams to join the 2025 New York State Age-Friendly Health Systems Action Community.

The goal of the Age-Friendly Health Systems (AFHS) Action Community is to engage hospitals and health systems to provide age-friendly care to all older adults by implementing evidence-based interventions across four elements of care, known as the “4Ms”: What Matters, Medication, Mentation and Mobility. Teams will join the Action Community to launch and rapidly spread the age-friendly movement across New York, aligning with the statewide goal that 50% of health systems become age-friendly.

The AFHS New York State Action Community will take place from January 2025 through October 2025. The Action Community is free of charge; some health systems may be eligible to apply for stipends to support this work.

44 HANYS New York healthcare providers’ achieved Age-Friendly Health System designation as Level 1 or Level 2 Age-Friendly Health Systems by the Institute for Healthcare Improvement. These providers participated in the HANYS-led New York State Age-Friendly Action Community 2024 cohort.

Recruitment for the Action Community will run through January 2025.

Funding is provided by the New York State Health Foundation, Health Foundation of Western and Central New York, Fan Fox & Leslie R. Samuels Foundation and New York Community Trust.
