Winners Announced for The John A. Hartford Foundation 2023 Business Innovation Award
USAging’s Aging and Disability Business Institute has awarded the Western New York Integrated Care Collaborative, Inc. (WNYICC) with The John A. Hartford Foundation 2023 Business Innovation Award, which recognizes effective partnerships between community-based organizations and health care entities.
Through its 25 contracts with health plans, WNYICC's more than 50 members help reduce barriers faced by older adults and people with disabilities in accessing medical care by addressing the health-related social needs of individuals living in western New York through a variety of programs and services, such as nutrition and diabetes self-management programs.
Runners-up for The John A. Hartford Foundation 2023 Business Innovation Award were the Columbus, Ohio based Central Ohio Area Agency on Aging (COAAA) for its creation of a simplified tool to screen patients for health-related social needs through its Social Determinant of Health Accelerator Program, and the Region IV Area Agency on Aging for its partnership with Corewell Health South to reduce the frequency of emergency department visits among older adults by using interventions that apply the 4Ms framework.
