JAMA Viewpoint: Hospital at Home and Transforming US Health Care Delivery
JAMA has published a Viewpoint article, "Hospital at Home and Transforming U.S. Health Care Delivery," on the opportunity to scale Hospital at Home and strengthen home-based care delivery in the United States.
Authored by leaders of the Hospital at Home Users Group, the article discusses the five-year extension of the Centers for Medicare & Medicaid Services Acute Hospital Care at Home waiver, which gives the field more time to scale Hospital at Home and collect additional data on clinical outcomes and costs.
The authors identify several areas that must be addressed to scale Hospital at Home, including quality assessment, workforce development, family caregiver supports, state and federal regulatory alignment, inclusion of observation-status patients, and continued innovation in technology and remote patient monitoring.
The article also explores how Hospital at Home can help support a broader home-based care ecosystem, with frail older adults, especially those who are homebound or experience preventable hospitalizations, identified as an appropriate initial target population. The authors note that Hospital at Home presents an opportunity to reimagine care delivery in a distributed, coordinated and patient-centered system of care in people's homes.
