NADRC Webinar: Finding the Way in Dementia Care - Use of Care Navigators for People Living with Dementia and their Caregivers
The National Alzheimer’s and Dementia Resource Center (NADRC) is holding a webinar February 6 on "Finding the Way in Dementia Care: Use of Care Navigators for People Living with Dementia and their Caregivers."
Care Navigators assist people living with dementia and their caregivers connect with needed resources and provide important insight and information about their clients to the full dementia team.
The webinar will talk about two programs using Care Navigators:
- CAlz Connect, an ACL-funded initiative through the California Department of Aging (CDA), offers dementia care management services to people living with dementia and their caregivers in Imperial, Marin, and Ventura counties, using the Care Ecosystem and HomeMeds evidence-based programs.
- OCCK, Inc. in Kansas is implementing the evidence-based Care Ecosystem in various rural communities with its ACL Alzheimer’s Disease Programs Initiative funding. Community Care Navigators help the Care Ecosystem team understand the day-to-day issues experienced by people living with dementia and their caregivers and provide needed services and supports.
