NADRC Webinar: Hospital2Home - Dementia Capable Care Transitions - Better Care, Better Outcomes
The National Alzheimer’s and Dementia Resource Center (NADRC) is holding a webinar on September 15, "Hospital2Home - Dementia Capable Care Transitions: Better Care, Better Outcomes."
Persons with cognitive impairment and their care partners experience daunting challenges receiving dementia capable healthcare and related community-based transition services during and after a hospital stay. This presentation will introduce participants to Hospital2Home, a care transitions model for people living with dementia with a hospital readmission rate of less than 1%.
The webinar will identify challenges caring for people living with dementia present to acute care systems, challenges people living with dementia and their care partners experience during and after a hospital stay, and the aim of Hospital2Home. Presenters include Jeffrey Klein, President/CEO of Nevada Senior Services Inc.
The webinar is part of the NADRC web seminar series, sponsored by the Administration for Community Living. It is being hosted by the American Society on Aging (ASA).
