National Committee for Quality Assurance: Person-Centered Outcome Measures Resource Center
The National Committee for Quality Assurance (NCQA) has updated its web resource center on person-centered outcome measures with new resources and a patient advocate video, "Person-Centered Outcome Measures Resource Center."
Person-Centered Outcome (PCO) measures assist clinicians and patients in setting, tracking and achieving personal health goals that focus on what matters most to the patient, an essential aspect of age-friendly care. The measures are designed to reduce unnecessary care, improve outcomes, and enhance communication.
PCO measures have gained traction in Medicare, behavioral health, and primary care settings. They are being tested in Dual-Eligible Special Needs Plans (D-SNPs), which have specific requirements for individualized care plans. Over the past decade, NCQA has developed these measures with support from The John A. Hartford Foundation, The SCAN Foundation, and The Gordon and Betty Moore Foundation.
The resource center includes a video with David White, a patient advocate, and resources on:
- Measuring What Matters
- Using PCO Measures
- Increasing Adoption
- Taking the Next Step
- Additional Resources (including PCO Approach Trainings, webinars and publications)
- Allies & Partners
